One in three OTR drivers has it. Most don't know until an examiner says the word. Here's how sleep apnea quietly ends careers — and how a machine the size of a lunchbox saves them.

You wake up more tired than when you laid down. Your wife says you stop breathing. You nod off at wheel-time you never used to. And then the examiner says the word: apnea. If you've heard it once, this article is for you.

What sleep apnea does to a driver

Every time your airway collapses, oxygen drops. Your body panics awake — 30, 60, sometimes 100 times a night. You never remember it. But you never get REM sleep either. That's why your reflexes at 3 p.m. feel like you're driving through molasses.

The DOT triggers for a sleep study

There's no automatic disqualifier, but examiners will refer you for a sleep study if you check three or more of these:

  • BMI over 33.
  • Neck circumference over 17" (men) or 16" (women).
  • Snoring reported by a partner.
  • Falling asleep during the day.
  • High blood pressure resistant to medication.
  • Age over 42.

The AHI number that matters

AHI is apnea-hypopnea index — events per hour. Under 5 is normal. 5-15 is mild. 15-30 is moderate. Over 30 is severe. Moderate and above almost always requires CPAP for CDL clearance.

The machine isn't the punishment. It's the reason you're still driving at 55.

Making CPAP work on the road

  • 12-volt DC adapter — no inverter needed. Plug into the cig lighter port.
  • Distilled water only. Buy the gallon jugs at Walmart.
  • Wipe the mask cushion every morning. It doubles the life.
  • Replace the filter monthly. It's a $5 job, not a service call.
  • Bring the SD card to every DOT physical — that's your compliance proof.

The compliance rule (the one that trips people up)
